Program Guidelines
- Fees are guaranteed for one year from the date of purchase.
- Membership rate is non-refundable after 30 days from initial purchase. If services are rendered during this
period, those fees will be deducted from membership refund. - Our plan is non-assignable to another person.
- One-year limit for completion of all services provided by the plan. Work started before the end of the plan year will be completed if the work can be finished within one month after the expiration of the plan year (at Doctor’s discretion)
- Benefits of the plan are only available in our office- this plan is not part of any other insurance or discount plan.
- Services cannot be filed to dental insurance.
- Plan benefits are not available with any other discount offer.
- This plan is available to uninsured patients only.
- Appointments are required.
- After hour emergency treatment is NOT included.
- All failed appointments and appointments cancelled without 24 hour notice are subject to a $30.00 missed appointment fee.
- Cannot be used in conjunction with Care Credit Financing.
- Discount is not applicable for oral sedation.
What’s Included
- Unlimited Exams (D0150 or D0120) each year from the date of the signed and paid contract.
- 2 Cleanings (D1110 or D4910) each year from the date of the signed and paid contract. All x-rays are included with my cleaning visits (D0272, D0273, D0274, D0330)
- 2 Fluoride Treatments (D1208) per year. *
- Annual Oral Cancer Screening
- Head & Neck Cancer Screening
- Complete Periodontal Evaluation
- 20% Smile Care Plan discount on minor restorative services: Composite Fillings and Simple Extractions
- 15% Smile Care Plan Discount on all major services: Endodontics, Periodontics, Prosthodontics and Complex Oral Surgery Services
- 10% Smile Care Plan Discount on Implant Services
- There are no discounts available for traditional denture services
